Resident Evil Requiem introduces one of Capcom’s most intricate puzzles yet, weaving together cryptic codes, multiple playthrough requirements, and unexpected in-game tasks like standing idle and flushing toilets. This elusive mystery challenges players to look beyond the surface and piece together clues scattered through the narrative and environment, making it a standout moment for the truly dedicated fanbase.

The Final Puzzle Walkthrough

Unlocking The Final Puzzle requires at least two full playthroughs: one to collect the necessary item to carry forward into the New Game Plus (NG+) and another to use that item to crack a complex code securing a mysterious safe. Here’s how to approach this layered challenge efficiently.

Step 1: Prep

Long before you meet the puzzle head-on, you’ll need to set the stage during the basement segment of the game. Here, you must wait as exactly 115 zombie bodies fall into a blood pool. This ties directly to a game file known as Subject 170’s record, hinting at this uncommon task as a prerequisite for the puzzle.

Stand by patiently for about 8 to 15 minutes, then trigger the draining mechanism of the pool. Importantly, avoid killing any zombies yourself; they must all be drawn into the shredder naturally. Afterwards, drain the second blood pool where the character Grace lands using a nearby lever. Success is confirmed when the prompt “A ton of blood flowed out” appears on screen.

Step 2: Learning the Letters

During the Care Center’s basement section, a chase sequence ends with Grace severing the hand of the relentless ‘The Girl’ monster. This severed hand is collectible and placed into your inventory, seemingly an odd inclusion at first since it doesn’t relate to core puzzles.

Yet this item is central to uncovering the game’s buried Easter egg. Stash it safely, as it will reveal hidden information when used at the right location.

Bring the Severed Hand to a blood analysis machine within the Care Center and complete the atom-matching puzzle presented, treating the hand as a sample. Unlike other analyses, this interaction doesn’t yield a crafting recipe. Instead, you’ll receive the cryptic message:

“Let’s play.”

GGC AAG AUA ACG UGU CAU

This sequence resembles RNA code and directs you to the Lead Researcher’s Office in the East Wing. There, the safe that previously held the Sun Quartz can be interacted with again, prompting you to input a new code — but with only three buttons labeled Sun, Moon, and Star. Decoding this RNA string is the next step.

To solve this puzzle, you must translate the RNA’s four different letters into one of these three celestial symbols, a process that requires some external pointer.

Step 3: Flushing the Toilet

Taking the RNA cipher in hand, head to the Care Center’s West Wing bathroom. Enter the second stall on the left and you’ll find a flushing toilet. Early in the game, you might have encountered an Investigation Report showing a corpse near a toilet with a bloodied number eight on the wall — this is your clue. You must flush this toilet eight times to continue progress in the puzzle.

Step 4: Converting the Letters into Symbols

Figuring out which RNA letters correspond to Sun, Moon, or Star might initially feel like a dead end, since their connections stretch beyond the Care Center. Thankfully, hints are cleverly hidden nearby to help.

One early insight is found on the blood analysis machine itself in the East Lobby’s Examination Room, where the letter G is associated with the number 150,000,000.

G = 150,000,000

This number signifies the average distance in kilometers from the Earth to the Sun, making all Gs in the RNA string correspond to the Sun symbol on the safe.

The next clue is more subtle and found in a small building adjacent to the Care Center. Here, completing a standard puzzle involving a helix inside a wine bottle is typical Resident Evil fare, but a nearby hourglass shaped like a helix grabs your attention during exploration.

By tilting this hourglass, you reveal another numerical hint:

U = 380,000

This matches the roughly 380,000-kilometer distance between the Earth and Moon, meaning all Us in the sequence represent the Moon symbol.

Further exploring this building, a file containing Grace’s blood report offers a final numeric clue on its last page:

C = 4.2 ly

C corresponds to the distance to the nearest star from Earth, about 4.2 light-years. Therefore, every C in the RNA code matches the Star symbol on the safe.

Step 5: Retrieving the Doll

Those initial 15 minutes of waiting and the multiple toilet flushes aren’t trivial; they manipulate the facility’s blood collection system. If done correctly, a discarded doll will appear near the stairs during the laboratory’s self-destruct sequence. This doll should be stored in your item box, as it’s crucial for the puzzle’s next stage.

Step 6: Removing A

With Sun, Moon, and Star decoded, the outstanding question is what to do with the letter A, also present in the RNA code. Unfortunately, this is beyond the immediate puzzle and requires further story progression, specifically reaching the end of Leon’s campaign.

After completing the Police Department section, you’ll pass through Kendo’s Gun Shop. While initially just a setting with broken bobblehead toys, it later becomes integral to the puzzle.

Following the intense battle with Mr. X, enter the nearby basketball court area. Here you’ll find a crate to break as well as a curious decapitated bobblehead that can be kicked around.

By nudging the head into the broken basketball hoop, you trigger the message:

“There’s no time for playing around”

With this clue in hand, return to Kendo’s Gun Shop to interact with the broken bobbleheads, unlocking yet another cryptic hint necessary to finally solve the puzzle.
